Individual Support

The Individual Support Program provides a variety of personal and social supports to individuals with developmental disabilties who live on their own or with their families. This is done by providing Personal Assistant Services, Temporary Residential Placement, Recreational Services, and/or Supported Employment Follow-Along.The purpose of our program is to increase an individual’s ability of self-care as well as help them interact in social activities in their community.

 Personal Assistant Services

The Personal Assistant Service provides assistance to individuals with life skills, daily living activities, health and safety issues and/or community involvement.  Once funding has been secured, a meeting is held with the individual and /or family members to obtain needed information for the service(s) goals to be set forth in the Individual Program Plan (IP).  The Director of the program gathers information regarding days and times services are to occur and staff is assigned appropriately. 

  Temporary Residential Placement

Temporary Residential Placement is provided at the Sullivan House Group Home in Sullivan, MO.  There is one bed in a private room in which a person can stay for up to two weeks.  Temporary Residential Placement can be used in emergency situations or if a family needs a vacation and their loved one needs 24 hour care.  This service provides quality care for older teens or adults with a developmental disability.
